In an effort to streamline your visit and minimize time spent in our office, we prefer that you complete all paperwork before coming to the office.

Core uses Klara to communicate and gather info. You will get a text message with links to fill out forms 1-2 days prior to your visit.

All required paperwork is also available as an e-file that can be signed electronically using your smart device or computer by visiting https://patient.klara.com/#/login.
